Kissito Healthcare Midland is a 114 bed skilled nursing facility with a focus on short-term transitional health care and rehabilitation services. From basic injuries and illnesses to the most medically complex cases, we provide our patients with the compassionate care and clinical services they need to achieve their greatest potential. We combine a comfortable healing environment with an innovative Collaborative Patient Care Pathway™ that utilizes interdisciplinary teams of physicians, nurse practitioners, licensed nurses and therapists. They work closely with the patients and their families to develop personalized care plans based on their needs, preferences and life experiences. The goal of the model is to educate and empower both the care giver and patient to take an active role in their plan of care both during and after their stay. This unique approach enables patients to return to their lives sooner and function at the highest level. After all, nothing is more important to us than making sure each patient makes a full recovery and is able to transition back to the comfort of their own home.

Should a patient not be able to transfer home and need more care, we do offer a long term care program to meet that need.

• IV therapy for antibiotics, hydration, pain management, total parenteral nutrition.
• Patient/caregiver teaching for management of new ostomies and feeding tubes.
